Abstract	Coronary heart disease mortality in South Carolina's rural Pee Dee area is among the highest in the nation. Residents of this area, aged 35 - 74 years, who were hospitalized with a diagnosis of acute myocardial infarction (AMI) or who died out-of-hospital with death attributed by nosologist to AMI during 1985 were identified from hospital and death certificate records. Information on personal risk factors, medical history, and access to medical care was obtained by informant interview. Characteristics are compared for individuals who died out-of-hospital of AMI (cases) and those who survived to be hospitalized with AMI (controls). Overall response rates were 81.8% among cases and 34.9% among controls, the latter resulting in small groups available for comparisons and low statistical power. None of the suspected risk factor variables significantly distinguished the two groups based on relative risk estimates (RR). Current smoking showed a RR of 6.0 among black males (lower 95% confidence limit (CL): 0.53). Ever smoking showed a RR of 4.0 for white males (lower 95% CL: 0.87). Among white females, history of diabetes was associated with a RR of 6.0 (lower 95% CL: 0.34), and history of hypertension showed a RR of 5.5 among black females (lower 95% CL: 0.23). Logistic regression identified race-sex group and Quetelet index of body mass as significant (p <.05) predictors of case or control status, with white men at less risk of out-of-hospital AMI death than other subjects. Cases and controls differed significantly (p <.003) on choice of medical care facility, with a higher proportion of cases preferring the doctor's office over the hospital emergency room for emergency care. Only 8.7% of cases, vs. 22.5% of controls (p =.07), would use ambulance service in case of emergency. 98.9% of controls and 77.8% of cases (p <.001) had telephones in residence.
