Abstract	In 1996, the FDA approved the use of Tissue Plasminogen Activator (t-PA) for the treatment of acute ischemic stroke (AIS). As of 2007, this treatment remains the only FDA approved medical therapy for this condition which results in high levels of death and disability, significant health care costs and major effects on society. The reasons for the slow adoption of this innovation relate to the stringent clinical conditions for its use, primarily the three hour window of opportunity after symptom onset and the comprehensive and costly array of medical systems needed to support thrombolysis. In October of2005, CMS doubled the reimbursement rate for the treatment of AIS with t-PA. This study evaluates the effect of this rate change on the adoption of the use oft-PA overall and by different types of hospitals, including those which are aggressive in their adoption of new practices, those who are more conventional in their practice style and those which are less likely to adopt new practices. The use rate was found to increase after the reimbursement change and did so more rapidly in stroke centers than in other hospitals.
