Abstract	Ultrastructural identification of a new structure within the glomerular basement membrane (GBM) called the lamina rara externa (lre) membrane has been successfully accomplished. Lre membranes lie parallel to the long axis of GBM ~170A from the epithelial plasmalemma. The structure measures ~120A in diameter bisecting individual pedicellar rays which emanate from epithelial foot processes and extend to the lamina densa. Identification of this structure is achieved by aortic retrograde perfusion with tannic acidglutaraldehyde fixative followed by prolonged staining of embedded tissue with bismuth subnitrate. Remarkable alterations occur in the organization and structural integrity of the lre membrane prior to the onset of proteinuria. Evaluation of six experimental renal disease models, namely puromycin aminonucleoside nephrosis, nephrotoxic serum nephritis, autologous immune complex disease, bovine serum album induced proteinuria, subtotal nephrectomy induced proteinuria and murine lupus nephritis seen in NZB/W F1 hybrid mice, has revealed loss of lre membrane integrit prior to the onset of proteinuria. Lre membranes proved a size selective barrier restricting horseradish peroxidase (m.w. 40,000: ae=55A) or catalse (m.w. 240,000: ae=52A) exclusively upon disruption of normal renal blood flow. Localization of anionic sites within the GBM at ~300A from the epithelia plasmalemma suggests a close relationship between the lre membrane function as a size and charge selective barrier.
