Abstract	Enteroadherent-enteropathogenic Escherichia coli produce diarrhea in man and animals several days following ingestion. This is unlike diarrheal illness caused by toxigenic or invasive E. coli which have incubation periods of 8-24 hours. Using the rabbit model for enteroadherent-enteropathogenic E. coli diarrhea, this study describes the localization of the diarrheal agent (RDEC-l strain) within the rabbit intestine during the interval preceding symptomatic illness. The results indicate that significant numbers of the RDEC-l strain associate specifically with the microvilli of M cells of the Peyer's patch lymph follicle epithelium within 4-8 hours post-orogastric inoculation. The RDEC-l strain bacterium then approaches the lumenal plasmallemna of the M cell and adjacent enterocytes producing a pattern of adherence identicle to that seen on enterocytes of the ileal, cecal, and colonic mucosae 7-14 days postinoculation. Two days postinoculation the numbers of RDEC-l strain become magnified, forming a bacterial microcolony over the apex of the lymph follicle epithelium. This bacterial microcolony serves as the source of bacteria which continuously seed the distal gut. The prolonged incubation period seen in the enteroadherent-enteropathogenic E. coli diarrheal illness caused by the RDEC-1 strain is explained by the necessity of the bacterium to first establish itself at the lymphoid follicle epithelium, increase in numbers, seed the distal gut, and finally reach a population size able to elicit diarrhea. The affinity of the RDEC-l strain for the lymph follicle epithelium of the rabbit intestine is due, at least partially, to the deficiency of goblet cell mucous and glycocalyx at the follicular epithelium. Surface factors, pili, present on the bacterium are also implicated in the initial adherence of the RDEC-l strain to the lymph follicle epithelium. A nonpathogenic bacterium which is able to synthesize and express RDEC-l strain pili adheres to the lymph follicle epithelium of the rabbit, while a similar bacterium which cannot produce the pili does not adhere. Antigens directed to the follicular epithelium would be more likely to elicit a secretory immune response. It is concluded that the affinity of the RDEC-l strain pili for the rabbit lymphoid follicle epithelium can be exploited to direct antigens to the follicular epithelium.
